will fill out the Membership Application.
- upon
acceptance, you will need to discontinue service with
the other Cellular Vendor.

may be subject to a disconnect fee.

at your request, the other Cellular Vendor will not
waive this fee, then you may elect to wait until
the end of your contract.

thereafter
your service will be changed to VERIZON WIRELESS
through RDI RateSavers.
